1. About these Terms
These Terms & Conditions ("Terms") govern your use of the Leafwise website, online services and booking functionality.
By accessing or using our website, submitting an enquiry or booking a service, you agree to these Terms.
"Leafwise", "we", "us" and "our" refer to Leafwise Clinic Pty Ltd, 79 536 634 970, trading as Leafwise.
2. Healthcare services
Leafwise provides access to healthcare services delivered by appropriately qualified healthcare practitioners.
The availability or description of a service on our website does not mean that a particular service, treatment or medication will be appropriate for you.
Clinical decisions are made by your treating healthcare practitioner following an appropriate assessment.
3. Website information is not medical advice
Information on the Leafwise website is provided for general informational and educational purposes only.
Website content does not constitute medical advice, diagnosis, a prescription, clinical assessment or a recommendation for any particular medication, treatment or healthcare service.
You should not rely on website information as a substitute for advice from an appropriately qualified healthcare professional who has assessed your circumstances.
4. Medical emergencies
Leafwise's website, enquiry forms, email and routine clinic communication channels are not emergency medical services.
For a medical emergency, call 000 or attend your nearest hospital emergency department.
5. Consultations do not guarantee treatment
Booking or attending a consultation does not guarantee a prescription, medication, procedure, referral or particular treatment.
Your treating healthcare practitioner will consider your medical history, current health, previous treatments and other relevant circumstances before discussing appropriate options.
All prescribing and treatment decisions remain solely with the treating healthcare practitioner and are made according to clinical judgement, professional obligations and applicable Australian laws and regulations.
6. Treatment outcomes
Healthcare and treatment outcomes vary between people.
Leafwise does not guarantee that any medication, therapy, procedure or healthcare service will achieve a particular outcome.
Treatments may involve risks, side effects, contraindications and limitations, which should be discussed with your treating practitioner.
7. Appointments
Appointments may be available face to face or via telehealth depending on the service, practitioner availability and clinical suitability.
You are responsible for providing accurate and complete information when booking and throughout your care.
8. Telehealth
Leafwise provides telehealth consultations for eligible patients and appropriate services.
You are responsible for having a suitable device, stable internet connection and reasonably private environment for your consultation.
You should provide accurate contact details and inform your practitioner of your physical location or the presence of another person during the consultation if requested.
If your connection is interrupted, your practitioner may attempt to reconnect or contact you using the telephone number you have provided.
Your practitioner may determine that an in-person assessment is required before care can safely or appropriately proceed.
9. Eligibility
Unless otherwise specified, Leafwise services are available to adults aged 18 years and over.
Some treatments and services may have additional eligibility requirements based on clinical, legal or regulatory considerations.
10. Prescriptions
Prescriptions are issued only where your treating healthcare practitioner determines that medication is clinically appropriate.
Where a prescription is issued, depending on the medication and service, you may be able to:
- receive an eScript;
- nominate a pharmacy of your choice;
- arrange dispensing through our onsite or associated pharmacy; or
- arrange medication delivery where available.
Prescription validity, repeats and dispensing requirements vary according to the medication and applicable requirements.
11. Pharmacy and medication delivery
Where pharmacy services are available, they may be provided by a separate pharmacy entity.
Medication pricing, availability, dispensing and delivery timeframes may vary.
If you choose delivery, you are responsible for providing accurate delivery details and following applicable medication storage and handling instructions.
Delivery timeframes may be affected by pharmacy processing, stock availability, location, courier services and circumstances outside our reasonable control.
12. Medication and side effects
Medication should be used according to the directions provided by your prescribing practitioner and pharmacist.
If your medication is not working as expected, you experience side effects, or you have concerns about your treatment, please contact the Leafwise clinical team.
Do not make significant changes to prescribed treatment without seeking appropriate clinical advice.
For severe symptoms or a medical emergency, call 000 or attend your nearest hospital emergency department.
13. Fees
Applicable consultation fees will be communicated during the booking process or before the relevant service is provided.
Unless expressly stated otherwise, consultation fees relate to the healthcare service or clinician time and do not necessarily include:
- medication;
- pharmacy dispensing;
- pathology;
- diagnostic testing;
- procedures;
- specialist or third-party services; or
- medication delivery.
14. Medicare
Medicare rebates can be processed by Leafwise for eligible consultations.
Eligibility for Medicare benefits depends on the consultation, service provided and applicable Medicare requirements.
A Medicare rebate is not available for every Leafwise service.
Where applicable, any consultation fee and anticipated Medicare rebate will be communicated as part of the booking or billing process.
15. Private health insurance
Leafwise does not process private health insurance claims directly.
Where applicable, Leafwise will provide you with a receipt that you can submit to your private health insurer.
Eligibility and rebate amounts vary between insurers, policies and services. You are responsible for confirming your eligibility directly with your private health insurer.
16. DVA
Leafwise does not currently provide services under DVA funding arrangements.
DVA cardholders may access Leafwise services as private patients, subject to the applicable consultation fees and eligibility requirements.
17. Payments
You are responsible for paying applicable fees associated with your appointment and services.
Payment may be required when booking, before your appointment or following the provision of a service.
Payments may be securely processed through Halaxy's integrated payment functionality or another payment gateway selected by Leafwise.
By providing payment information, you authorise the applicable payment provider to process payments in accordance with the fees and booking information presented to you.
18. Cancellation and rescheduling
If you need to cancel or reschedule your appointment, Leafwise requires at least 24 hours' notice.
Appointments cancelled or rescheduled more than 24 hours before the scheduled appointment will not incur a cancellation fee.
Appointments cancelled or rescheduled within 24 hours of the scheduled appointment may incur a fee equal to 50% of the consultation fee.
If you do not attend your appointment without providing notice, the full consultation fee may be charged.
For telehealth consultations, failure to join the appointment or respond when your practitioner attempts to contact you may be treated as a missed appointment.
We understand that genuine emergencies and exceptional circumstances occur. Cancellation fees may be waived at Leafwise's discretion in appropriate circumstances.
If Leafwise needs to cancel or reschedule your appointment, we will endeavour to contact you as soon as reasonably possible and arrange an alternative appointment. You will not be charged a cancellation fee where Leafwise cancels your appointment.
19. Late arrivals
If you arrive late, your practitioner will make reasonable efforts to see you within the remaining appointment time.
If there is insufficient time to safely or appropriately complete your consultation, your appointment may need to be rescheduled and a late cancellation fee may apply.
20. Refunds and Australian Consumer Law
Refund requests will be considered according to the circumstances of the relevant service and applicable Australian law.
Nothing in these Terms excludes, restricts or modifies any consumer guarantee, right or remedy that cannot lawfully be excluded under the Australian Consumer Law.
21. Your responsibilities
Safe and appropriate healthcare depends on your practitioner having accurate information.
You are responsible for providing information that is accurate and complete to the best of your knowledge, including relevant information about:
- your medical history;
- current medications;
- allergies and adverse reactions;
- diagnoses;
- previous treatments;
- other healthcare professionals involved in your care; and
- changes in your health or circumstances that may affect treatment.
You should tell your practitioner if you do not understand information or instructions provided to you.
22. Working with your existing healthcare providers
Leafwise can work alongside your existing GP, specialist, pharmacist and other healthcare professionals.
With your consent and where clinically appropriate, information may be shared with members of your existing healthcare team to support continuity and coordination of care.
23. Work injuries and WorkCover
Where Leafwise provides healthcare relating to workplace injuries or workers compensation claims, additional information may be required regarding your injury, employer, insurer and claim.
Any medical certificate, report, treatment recommendation or return-to-work advice will be based on the practitioner's clinical assessment and applicable requirements.
Leafwise cannot guarantee that a workers compensation claim, treatment request or associated expense will be accepted by an employer, insurer or relevant authority.
24. Alternative medicines and other services
Leafwise provides a range of healthcare services, which may include alternative medicines, cosmetic and rejuvenation procedures, skin health, weight and diet management, work injury and WorkCover services and peptide therapy.
Certain services may be subject to additional clinical assessments, eligibility criteria, consent requirements, risks, regulatory restrictions and follow-up requirements.
Information about these services on the website does not establish that a treatment is suitable for you.
25. Cosmetic and rejuvenation procedures
Cosmetic and rejuvenation procedures may involve specific risks, contraindications, recovery considerations and expected outcomes.
Suitability is determined following an appropriate consultation and assessment.
No particular cosmetic result can be guaranteed.
Where additional informed consent documentation is required, this must be completed before treatment proceeds.
26. Peptide therapy
Peptide therapies are not appropriate or available for every patient.
Availability and suitability depend on the specific treatment, your clinical circumstances, applicable prescribing requirements and your treating practitioner's professional judgement.
Information on the Leafwise website about peptide therapy is general and does not constitute a recommendation or guarantee of treatment.
27. Weight and diet management
Weight management services may include medical, nutritional, lifestyle and other clinically appropriate strategies.
Treatment recommendations depend on your circumstances and clinical assessment.
No particular amount or rate of weight loss can be guaranteed.
28. Respectful behaviour
Leafwise is committed to providing a safe and respectful environment for patients, practitioners, staff and visitors.
Threatening, abusive, discriminatory, harassing or otherwise inappropriate behaviour will not be accepted.
Where lawful and appropriate, Leafwise may decline or discontinue non-emergency services where behaviour creates an unreasonable safety or operational risk.
29. Third-party services
Leafwise may use or interact with third parties including pharmacies, pathology providers, payment processors, booking and practice management platforms, telehealth providers, couriers, insurers and other healthcare providers.
Where a third party provides a separate service directly to you, that provider's terms, policies and fees may also apply.
30. Third-party links
The Leafwise website may contain links to external websites.
We do not control those websites and are not responsible for their content, security, availability or privacy practices.
31. Intellectual property
Unless otherwise stated, website content including text, branding, graphics, images, design and other materials is owned by or licensed to Leafwise and protected by applicable intellectual property laws.
You may use the website for personal, non-commercial purposes.
Website content must not be reproduced, republished, modified, distributed or commercially exploited without prior written permission, except where permitted by law.
32. Website availability and accuracy
Leafwise takes reasonable steps to maintain accurate and current website information but does not guarantee that the website will always be uninterrupted, error-free or completely current.
Healthcare information, services, regulatory requirements, fees and practitioner availability can change.
We may update, suspend or modify website content and functionality from time to time.
33. Limitation of liability
To the maximum extent permitted by law, Leafwise is not responsible for loss arising solely from reliance on general information published on the website instead of obtaining appropriate professional healthcare advice.
Nothing in these Terms excludes, restricts or modifies liability, consumer guarantees or other rights that cannot lawfully be excluded.
34. Privacy
Our collection, use, storage and disclosure of personal and health information is governed by our Privacy Policy and applicable Australian privacy and health information laws.
35. Electronic communications
Website forms, email and other routine electronic communications should not be used for urgent medical matters.
Messages may not be reviewed immediately.

36. Changes to these Terms
Leafwise may update these Terms from time to time to reflect changes to our website, services, policies, providers or legal obligations.

37. Governing law
These Terms are governed by the laws of the Australian state or territory in which the Leafwise operating entity is established and any applicable Commonwealth laws.
